As part of our standard procedures, and even more so during this period of heightened awareness, all equipment is thoroughly cleaned and disinfected. We also follow additional precautions by ensuring that the waiting room, common areas and all treatment areas are properly sanitized and disinfected. We have continued these procedures as well as increased the frequency of disinfection to ensure the health and safety for everyone. Furthermore, we are adjusting our patient scheduling to reduce shared space with others present at common times.
In consideration of the aforementioned, prior to your arrival for an appointment we ask you to fill out our Health Assessment Survey Begin Health Assessment Now, prior to requesting an appointment. This is our temporarily office policy and is mandatory for all patients of the clinic. After your survey is filled out and reviewed by the doctor, your appointment will be confirmed. This extra step, will ensure more monitoring and maintaining a healthy and safe environment for patients to be seen . Based on your assessment of possible exposure, we ask you to call us to cancel the appointment and re-schedule for a later date. We appreciate your understanding as this procedure aims to minimize exposure to other patients, their families, and our staff.
